Baker Elementary School Nurse: Elizabeth Phillips
(Grades K-3)
139 W. Maple Avenue Moorestown, NJ 08057 (856) 778-6630 extension 15049 Please notify school when your child will be absent.
ATTENDANCE - If your child will be absent or late, call the Health Office between the hours of 8 and 9:00 AM. We must account for all students every day. If your child is absent and a call has not been received, the school will contact you. Here is a brief outline of the health services available for students:
SCREENINGS
Every student is screened at the beginning of each school year. This includes height, weight, (Body Mass Index) vision, hearing, blood pressure, and a check for head lice. Parents are notificed if a follow-up is needed by your family physician or pediatrician. Since a child's vision or hearing can change in a short period of time, please let me know if you would like your child checked at any time during the school year.
EMERGENCY FORMS
Every parent or guardian will receive an emergency form to fill out every year. Please keep this form updated. Notify the school if there are any changes in addresses or telephone numbers so that you can be reached in your child is ill or hurt, or if there is an early school closing.
Our Board of Education policy and the N.J. Nurse Practice Act specify that if a child needs prescription or non-prescription medication in school, the parent or guardian must provide a written doctor's note with the child's name, dose, reason and time of administration. Forms are available in the nurse's office. The medication must be in the pharmacy container, and must be brought in by the parent. Parents may come in and administer medication to their children at any time.
IMMUNIZATIONS
Childhood immunizations have prevented more illness and saved more lives than any other public health measure in history. The N.J. Department of Health regulation specify that a child cannot enter school without up-to-date vaccines. If your child receives any immunizations, please let the nurse know so your child's health records can remain current. PHYSICAL EXAMINATIONS: Physician Report Form
Physical examinations are required on all kindergarten and new students. Forms can be obtained in the Health Office. Our school physician is also available for physical examinations for students who do not have a medical home.
HEALTH EDUCATION
All classes in grades 1-3 receive health classes on a weekly basis. These classes include lessons on nutrition, safety (fire, car, bus, pedestrian, water), dental health, healthy habits (exercise, sleep, germs, cleanliness), body systems, basic first aid, and substance abuse (drugs, alcohol, and cigarettes).
HEALTHY HABITS
Breakfast - A study reported in the Journal of School Health concluded that students who skip breakfast are less attentive, have higher cholesterol levels, and have lower scores on standardized tests. Tardiness and absences are markedly lower among breakfast-eaters.
Bedtime - Children thrive on routine, so it is important to establish a bedtime routine.
Attendance - If your child will be absent or late, call the Health Office between the hours of 8 and 9:00 AM, or leave a message on the voice mail. We must account for all students every day. If your child is absent and a call has not been received, the school will contact you. A note is also required when your child returns to school. You may also leave an e-mail message.
It is my pleasure and honor to work with you and your children. Please feel free to contact me with your thoughts and concerns.
